A student who is returning to or entering school with durable medical equipment (crutches, wheelchair, canes, splints, etc.) must present documentation from a physician that includes the following:

1. That the use of the DME has been authorized by the physician.

2. The diagnosis and duration of the use of the DME.

3. That the student has received training in the use of the DME.

4. Anticipated length of time the student will be excused from PE and/or recess.

The student will not be allowed to attend classes or school events until this documentation is received.

In addition, students returning to school after surgery, hospitalization, receiving a cast, stitches, or orthopedic appliances require a doctor’s note which states the following:

1. The date the student is cleared to return to school.
2. Any restrictions to school participation.
3. Date of follow-up appointment.

Equipo médico duradero

Un estudiante que regresa o ingresa a la escuela con equipo médico duradero (muletas, silla de ruedas, bastones, férulas, etc.) debe presentar documentación de un médico que incluya lo siguiente:

1. Que el uso del DME haya sido autorizado por el médico.
2. El diagnóstico y duración del uso del DME.
3. Que el estudiante haya recibido capacitación en el uso del DME.
4. Período previsto de tiempo que el estudiante estará excusado de educación física y/o recreo.

No se permitirá al estudiante asistir a clases o eventos escolares hasta que se reciba esta documentación.

Además, los estudiantes que regresan a la escuela después de una cirugía, hospitalización, yeso, puntos o aparatos ortopédicos requieren una nota del médico que indique lo siguiente:

1. La fecha en que el estudiante tiene autorización para regresar a la escuela.
2. Cualquier restricción a la participación escolar.
3. Fecha de la cita de seguimiento.
